Appointments Please arrange for your referring physician to complete the referral form available on our website (see Patient Referral Form) and fax it to us along with any relevant MD notes, MRI reports or CT scans. After the physician has reviewed/approved the request, you will be contacted by one of our staff to schedule your office visit. In most cases, the first visit will be an evaluation and not a procedure. We pride ourselves in getting new patients scheduled for their first visit very quickly (within a week or few days for the most part).
To help us provide you with the best of care, please bring recent MRI reports (very important) and MRI films if available. Additionally, we ask that you bring an updated list of medications that you are currently taking. If you have been treated by a pain management physician previously, please attempt to bring any medical records from those treatments to your office appointment.

Insurance Comprehensive Pain Management of Central Connecticut accepts most insurance plans, including PPO's, HMO's, Medicare, Workers Compensation, Motor Vehicle Insurance and Attorney Letters of Protection.
Payment Cash, Checks, or Credit Cards (Mastercard, Visa, Discover and Amex) are accepted for your payment. Co-payments are due at the time of visit.
