Request a Consultation

If you would like to be contacted by our office to arrange for a consultation with Comprehensive Pain Management, please complete the Request a Consultation form.

After your submission has been reviewed you will be contacted by one of our staff within two business days to schedule your office visit.   In most cases, the first visit will be an evaluation and not a procedure.  We pride ourselves in getting new patients scheduled for their first visit very quickly (within a week or few days).  We do reserve procedure slots so that we can schedule patient injections relatively soon after the first appointment. 

To help us provide you with the best of care, please bring recent MRI reports (very important) and MRI films if available. Additionally, we ask that you bring an updated list of medications that you are currently taking. If you have been treated by a pain management physician previously, please attempt to bring any medical records from those treatments to your office appointment.

We accept the following payment arrangements:

Insurance
Comprehensive Pain Management of Central Connecticut accepts most insurance plans, including PPO's, HMO's, Medicare, Workers Compensation, Motor Vehicle Insurance and Attorney Letters of Protection. Unfortunately, we do not accept Medicaid. 

Payment
Cash, Checks, or Credit Cards (Mastercard, Visa, Discover and Amex) are accepted for your payment.

Co-payments are due at the time of visit.

You will need to complete and bring the following forms with you for your first visit with our office.   Please download, complete and sign each form.  Your first visit will last about 40-45 minutes and will be an evaluation, not a procedure.

What to Bring for your First Visit:
  • Current Insurance Card
  • Photo ID
  • Completed Paperwork  (see above)
  • Co Pay (if applicable)
  • Recent MRI reports and/or CD’s
  • List of any meds you’ve tried for pain in past (and doses)

If you are unable to keep your appointment with us, please contact us at 860-793-0500.  We request at least 2 days advance notice for new appointment cancellations.  Failure to provide advanced cancellation notice may prevent you from being rescheduled for a future appointment.
